You have until March 31, 2018 to submit claims incurred through December 31, 2017 in order to exhaust your HRA balance.

Claims with a date of service after December 31, 2017 will be denied.

  • Amounts paid towards deductible or coinsurance
  • Prescription drugs
  • Vision expenses (e.g., eye exam, eye glasses, and contact lenses)
  • Dental and orthodontia expenses
  • Medical supplies and equipment (e.g., bandages, crutches, and diabetic supplies)

Ways you can be reimbursed

From the home page, go to VISIT Spending Accounts > Common Requests > Enter New Claims  to upload your itemized receipt or EOB.

  • Fax - log on to BenefitsConnection www.verizonbenefitsconnection.com  go to VISIT Spending Accounts > I Want To > View Forms > Health Care Spending Account Claim Form. Print the form and complete it according to the instructions. Fax it with your itemized receipt or EOB to 855-785-3471
  • Mail - log on to BenefitsConnection www.verizonbenefitsconnection.com go to VISIT Spending Accounts > I Want To > View Forms > Health Care Spending Account Claim Form. Print the form and complete it according to the instructions. Mail the form with your itemized receipt or EOB to:

LOST HRA

Go to BenefitsConnection> Visit Spending Accounts> Common Requests > Manage Debit Cards to order new ones. They should make sure they mark the old one as lost or stolen as well. Verizon Benefits Center 855-489-2367
