2015 ACTIVE ANNUAL ENROLLMENT INFORMATION

 

October 21 to November 4, 2014

 

(NOTE:  Please keep in mind that Mid-Atlantic members can make changes up until 12/31/14)

 

If you take no action, your current elections for medical, dental, vision, life insurance, spending accounts, tobacco/non-tobacco user designation and Health Assessment credit will automatically carry over for 2015, unless otherwise noted within the “Annual Enrollment 2015” brochure being mailed to members.

 

Confirmation of Coverage Statement 

 

A confirmation statement will no longer be automatically sentto your home address on file after Annual Enrollment. It is advised you request a copy of your Confirmation Statement by calling the Verizon Benefits Center (VBC) at 1-855-489-2367 or by logging onto www.verizonbenefitsconnection.com and printing out a copy.  

 

Health Assessment Completion Credit — Up to $100

 

If you are currently receiving the credit for previously completing the online Health Assessment, the credit will automatically apply for 2015.  

 

If you are not currently receiving the Health Assessment credit,complete the Health Assessment by December 10, 2014 to receive a $100 credit toward your annual medical plan contributions for 2015.

Completion of the Health Assessment after December 10, 2014 but before December 31,2014 may delay your full credit.  The Health Assessment can be completed by logging into www.verizonbenefitsconnection.com.

 

Non-Tobacco User Credit — Up to $600

 

If you are already receiving credit for completing this incentive, no further action is required.  The credit will be applied to your 2015 coverage.  If you have not yet completed a tobacco cessation program or updated your status, please refer to your 2015 Annual Enrollment brochure that will be mailed to you.

 

Please check your Annual Enrollment Brochure for changes in premiums, deductibles, and out of pockets expenses.
