- Spousal Surcharge-Spousal Surcharge of $40/month DOES NOT apply if your spouse or partner meet ANY one of the following seven criteria:
1. Your spouse or same sex domestic partner (SSDP) is not employed
2.Your spouse or SSDP is employed and enrolls in their employers coverage (Coordination of benefits would take effect between both plans)
3. Your spouse or SSDP is employed but his/her employer does not offer medical coverage
4.Your spouse or SSDP is not eligible to receive medical coverage under the employers plan
5.Your spouse or SSDP gross base wage on an annual basis is $25,000 or less
6. Your spouse or SSDP would be required to make an annual contribution to the medical premium of $900 or more for their employer’s coverage
7. Your spouse or SSDP is a Verizon associate
