Beginning October 1, 2009 members may call the Verizon Benefits Center (877-489-2367) to request the Health Plan Comparison Charts (HPCC) for the health plans that are available to them, effective January 1, 2010.

fill in city  or county, then state
Type of provider (ex: medical provider)
Provider type (ex: primary care physician)
Plan: select Open Access- Choice POS II (this is the MCN)
